If you have a referral, please bring this along to your initial appointment, along with any relevant reports.
We accept referrals for GP managed Enhanced Primary Care Plans. You will need to bring a copy of this along to your initial appointment or have it emailed to us prior to your appointment. Please note, gap payments are applicable.
